Student-Athlete Forms

As a National Junior College Athletic Association (NJCAA) Region XV member college, FIT Athletics places an emphasis on complying with all applicable NJCAA , Region XV, FIT Institutional, and FIT Athletics rules, regulations and guidelines. 

Please note that all requirements and information appearing below are subject to change.

Completion of student-athlete forms #1-5 is required for all student-athletes prior to trying out or practicing for the first time.

Medical Clearance  
*Please note, only the following 4-page form will be accepted as medial clearance for FIT Athletics purposes. Doctor's notes / alternative forms can not be accepted in place of the following physical exam form:

Instructions:
Print out the form and fill out ONLY pages 2 & 4 (be sure to answer all yes/no questions)
Bring or send all 4 pages of the form to your doctor so they can complete pages 1 & 3.
Prior to tryouts or practicing for the first time, upload all 4 completed pages to the FIT Health Services Portal or bring to FIT Health Services in room A401 (Dubinsky Student Center 4th Floor)
FIT Health Services will then review your submission and report athletics participation clearance status to the Athletics Department

Potential student-athletes are strongly encouraged to seek athletic participation clearance from their own doctor who is familiar with their health and family history.  However if necessary, FIT Health Services can conduct physical exams for potential student-athletes, but an appointment is required and might not be available in advance of your first tryout / practice date.  Appointment availability depends on doctors' schedules and the amount of students seeking appointments, which at peak times like the beginning of each semester, can be very high.   A third option would be to consider getting your physical exam completed at a local walk-in clinic.  Mt Sinai Doctors, CityMD or CVS Minute Clinics are some brands with many locations throughout the NYC area.

3 - Student-Athlete Guidelines
Print out, sign, and hand in to your coach.

4 - Student-Athlete Eligibility Affidavit
Print out, complete, and hand in to your coach.

5 - Athletics Waiver / Assumption of Risk
Print out, sign, and hand in to your coach.

Student-athletes making an official roster in any NJCAA sport must also complete student-athlete form #6 prior to competing for the first time. 

6 - NJCAA Student-Athlete Registration Form
** Do not fill this out until directed to do so by your coach or the Athletics Department**
Fill out and submit online.
